Why do internal communications, training, and operations tools become disconnected at work?

They drift apart because they were bought one urgent problem at a time. A safety incident buys a communication tool. An audit finding buys a learning system. Why integrations alone do not fix it

An integration moves data between two records. It does not make them one record. After the integration, the learning system still holds its own copy of the employee, the communications tool still holds its own, and the two disagree the day after a transfer. How to move from fragmented to aligned without a rip-and-replace
