Seven types: injury, near-miss, property damage, environmental, security, vehicle accident, and equipment failure. Each carries a severity of low, medium, high, or critical, and high/critical or injury incidents route into an investigation workflow.
Yes. Incidents and observations both have an anonymous flag, and there is a branded public safety portal at /safety where anyone can report a hazard, near-miss, incident, or suggestion without logging in. Anonymous submissions still capture type, category, description, and location.
Safety Hub maintains the OSHA 300 log, 300A annual summary, and 301 individual injury report, generates 300/301 as PDFs (or CSV for the 300 log), and lets you upload your own fillable PDF form templates. It also calculates TRIR and DART, and provides a Canadian WCB dashboard with LTIR and total injury rate by province. Electronic submission to OSHA's ITA is not yet built — you generate and file the forms yourself.
Corrective actions attach to an incident with one of eight types (immediate, short-term, long-term, preventive, training, policy change, equipment, environmental), a due date, and an assignee. They move through pending, in-progress, and completed, and overdue actions surface on the dashboard and in reminders.
Define certification requirements by role, location, department, or job title. Safety Hub aggregates held certifications from Skills, the Training app, and an external LMS (via Training Connect), flags expiring-soon and expired credentials, and shows gaps by employee or requirement. It surfaces the gaps but does not create training enrollments — re-enrollment happens in the Training app.
The Ask AI agent answers questions about incidents, observations, toolbox talks, and certifications, and can file an incident or observation directly from chat. Separately, autonomous monitoring detects stalled investigations and unassigned high-severity incidents and nudges the right managers — governed by a trust level, daily action limits, and automatic pause if error rates climb.
Safety Hub is the EHS system of record for incidents, investigations, observations, toolbox talks, certifications, and OSHA/WCB recordkeeping. Lone Worker handles real-time check-ins and SOS escalation for solo workers and hands an incident off to Safety Hub when an alert resolves as one. Inspections runs safety audits and checklists; its open and overdue corrective actions surface on the Safety Hub dashboard when both apps are enabled.
Safety Hub requires a license and is opt-in per tenant — an administrator enables it for your business from the Apps Marketplace. It is disabled by default until then. Contact your MangoApps administrator about turning it on.
