Anchor Identification and Installation

• Anchor type matches approved design
• Anchor and mounting hardware are securely installed
  Verify no looseness, movement, missing components, or visible separation at the mounting interface.
• Fasteners present and intact
  Check for missing, damaged, stripped, or substituted fasteners; verify locking devices or torque retention features where applicable.
• Roof substrate and structural attachment show no visible distress
  Inspect for cracking, deformation, water intrusion, rot, corrosion, or other signs that the substrate or structural member may have been compromised.

Condition and Serviceability

• Anchor shows no deformation, cracks, or weld damage
  Inspect all visible anchor components for bending, elongation, cracking, broken welds, or other structural damage.
• Corrosion or deterioration within acceptable limits
  Check for rust, pitting, coating failure, UV degradation, or other deterioration that could affect performance.
• Anchor label or identification markings legible
  Verify manufacturer identification, capacity rating, installation date, and inspection label are present and readable.
• Anchor is free of obstructions and accessible for use
  Confirm the anchor can be reached and used without removing permanent barriers, debris, or stored materials.
• No evidence of unauthorized modification or repair
  Look for field welding, drilling, cut components, non-approved replacement parts, or other non-conforming alterations.

Load Test, Recertification, and Compliance

• Annual inspection completed by qualified person
• Load test or recertification performed when required
  Document whether a load test, recertification, or visual-only inspection was performed based on the anchor manufacturer's instructions, site program, and any reroofing or renovation work.
• Inspection result meets OSHA / ANSI Z359 requirements
  Confirm the anchor remains suitable for continued service based on the inspection findings and applicable manufacturer and site requirements.
• Date of last reroofing or roof renovation recorded
  If reroofing or renovation occurred, record the date to support recertification tracking.

Deficiencies and Corrective Actions

• Deficiencies documented with corrective action
  Record all deficiencies, non-conformances, and required corrective actions, including removal from service if applicable.
• Anchor removed from service if unsafe
  Use this item when the anchor is unsafe, damaged, or non-compliant and must be tagged out until repaired or replaced.
