Coverage Dates and Status

• Effective date is on or before today
• Expiration date is after today
• Coverage is active with no lapse indicated
• Cancellation or non-renewal notice disclosed, if any

Auto Liability Coverage

• Auto liability coverage listed on COI
• Auto liability limit meets brokerage minimum
• Policy form or coverage description supports motor carrier operations
• Any auto liability exclusions or restrictions reviewed

Cargo Coverage

• Cargo coverage listed on COI
• Cargo limit meets load or brokerage minimum
• Cargo coverage applies to freight type being moved
• Deductible or self-insured retention reviewed

Exceptions and Approval

• Deficiencies or missing endorsements documented
