- Representative fuel sample retained for each applicable delivery
- Sample retention period meets or exceeds 12 months or until substantially consumed, whichever is longer
- Sample container is sealed and shows no evidence of tampering or leakage
- Sample label includes fuel grade, date of collection, vessel or facility name, and unique identifier
- Sample storage location is secure, dry, and protected from unauthorized access
- Number of representative samples reviewed
Findings, Corrective Actions, and Sign-Off
- Deficiencies documented with clear reference to the affected record or sample
- Corrective actions assigned with owner and due date
- Inspector comments and evidence uploaded
- Inspector signature
- Responsible party acknowledgment
