Suspension and Debarment (SAM.gov) Verification Log

Log to document SAM.gov verification that a subrecipient or contractor is not suspended, debarred, or excluded before award under 2 CFR 180.300.

Verification Record

• Verification Date
  Date the SAM.gov check was completed.
• Entity Type
  Select the type of organization being checked.
• Entity Name
  Legal name of the organization being verified.
• UEI or Other Internal Identifier
  Enter the UEI if available, or an internal identifier used for audit trail purposes. Do not collect SSN or other unnecessary PII.
• Award, Contract, or Subaward Reference
  Optional reference number for the related procurement or award action.

SAM.gov Search Details

• Search Method
  Select the method used to verify suspension, debarment, or exclusion status.
• Search Result Status
  Record the outcome of the verification.
• SAM.gov Record URL
  Optional link to the SAM.gov record or search results used for verification.
• Verification Notes
  Brief notes describing the search terms used, any clarifications, or why the result was unresolved.

Exception Review

• Exception Type
  Select all reasons that apply to the exception or unresolved result.
• Follow-Up Required Before Award?
  Indicate whether additional review is needed before proceeding.
• Assigned Reviewer
  Name or role of the person responsible for resolving the exception.
• Follow-Up Due Date
  Target date for completing the exception review.

Reviewer Attestation

• Reviewer Name
  Name of the staff member who completed the verification.
• Reviewer Title or Role
  Optional job title or role for the audit trail.
• Attestation
  Required confirmation that the verification was completed and documented.
