Cleanliness and Housekeeping

• Floors visibly clean and free of standing water, debris, and hair buildup
• Benches, counters, and high-touch surfaces are visibly clean
• Trash receptacles are not overflowing and lids/openings are sanitary
• Showers, sinks, and drains are free of visible buildup or blockage
• Odors are within normal facility conditions and do not indicate unsanitary conditions
• Cleaning tasks or custodial follow-up needed

Supplies and Fixture Readiness

• Soap dispensers are stocked and dispensing properly
• Paper towels or hand-drying supplies are available at usable levels
• Toilet paper is stocked in all accessible stalls
• Sanitizer, hygiene products, and other posted supplies are available as required by site SOP
• Locker doors, benches, hooks, and other fixtures are intact and functioning

Hazard Removal and Safety Conditions

• No slip, trip, or fall hazards present in walking paths
• Wet floor signs are placed when needed and removed when no longer required
• Broken glass, sharp objects, or other foreign hazards removed from the area
• Lighting is adequate for safe use of the locker room
• Emergency exits and egress paths are unobstructed

Youth Supervision and Privacy Compliance

• Youth supervision policy is being followed in the locker room
• No one-on-one adult and minor interaction observed in restricted areas
• Privacy expectations are maintained at showers, changing areas, and restroom entrances
