• Access to anchor is unobstructed and safe for inspection
• Anchor identification label or marking is legible
• Roof surface around anchor is free of trip, slip, or fall hazards

Anchor Physical Condition

• Anchor shows no visible cracks, bends, deformation, or missing components
• Fasteners, welds, and base plate are secure and intact
• No visible corrosion, rust-through, or material deterioration affecting strength
• Roof membrane, flashing, and waterproofing around anchor are intact
• Anchor has not been modified, painted over, or altered since last inspection

Load Test or Qualified Visual Inspection

• Inspection method used
• Inspection result meets manufacturer and site requirements
• If load tested, recorded test load
  Record the applied test load if a load test was performed. Verify against the site program, manufacturer instructions, and applicable OSHA criteria.
• Load test or inspection documentation attached

Deficiencies and Corrective Actions

• Any deficiency or non-conformance identified
• Anchor removed from service pending repair or replacement when required
• Corrective action assigned with responsible person and due date
