Safety Preconditions and Test Readiness

• Lockout-tagout applied where required and verified by qualified person
• Arc-flash boundaries, shock boundaries, and PPE selected for the task
• Test equipment calibrated and within current calibration date
• Secondary circuits isolated or test switches positioned to prevent unintended trip
• Trip circuit supervision and alarm status verified before injection

Relay Identification and Visual Condition

• Relay model, serial number, and firmware/version recorded
• Relay enclosure, terminal blocks, and wiring show no visible damage or overheating
• CT/PT ratios and polarity match the approved drawings
• Target flags, LEDs, and event indications cleared before test start

Pickup, Timing, and Trip Verification

• Overcurrent pickup value verified against setting sheet
• Measured pickup matches expected setting within tolerance
• Measured operating time recorded
• Measured operating time within coordination study tolerance
• Trip output verified to correct breaker, contactor, or lockout relay
• Trip circuit continuity and end-to-end trip path verified
• Additional protection elements tested

As-Left Settings and Results

• As-left relay settings match approved coordination study
• Final event report or test printout attached
• Test results summary

Deficiencies and Corrective Actions

• Any deficiency or non-conformance identified
• Corrective action documented for each failed or out-of-tolerance item

Sign-Off

• Inspector signature
• Reviewer / supervisor signature
