• No exclusion screening hits or unresolved matches are present in the file

Training and Certification

• CPR certification on file and current
• First aid certification on file and current
• PASS training completion documented
• Training expiration dates are tracked for renewal

Audit Readiness and Corrective Actions

• All deficiencies are documented with clear corrective actions
• Missing or expired documents have a follow-up owner and due date
• File is ready for broker or Medicaid audit without unresolved non-conformances
