Exterior Entry and Access

- Entry door can be opened and closed without excessive force
Interior Walking Surfaces and Fall Risks
- Walkways are clear of clutter, cords, and loose items
- Floor surfaces are even and free of loose rugs or curled edges
- Slip resistance concerns noted on hard flooring or wet-prone areas
- Furniture arrangement allows safe passage with mobility aids
- Lighting levels in main circulation areas are sufficient
Bathroom Safety
- Grab bars are installed securely at toilet and bathing areas
- Tub or shower entry is safe for current mobility level
- Non-slip surface or bath mat is present and in good condition
- Toilet height is appropriate for safe transfers
- Shower chair, handheld shower, or other bathing aid recommended if needed
Kitchen and Daily Task Safety
- Frequently used items are stored within safe reach
- Counter and sink access supports safe standing or seated use
- Appliance controls are readable and usable by resident
- Sharp objects, hot surfaces, and chemical storage present avoidable hazards
Bedroom, Stairs, and Emergency Readiness
- Bed height supports safe transfers
- Path from bed to bathroom is unobstructed and well lit
- Stairs have secure handrails and consistent step conditions
- Smoke and carbon monoxide alarms are present and functional
- Emergency exit route is identified and accessible
