Before Photo Evidence

• Before photo captured for the deficiency
• Before photo shows the full context of the area
• Before photo clearly shows the specific deficiency
• Before photo is dated or linked to the visit record
• Before condition description entered
• Immediate risk or impact noted

Corrective Action Tracking

• Corrective action description entered
• Work completion date recorded
• Work completed by identified person or vendor
• Interim controls maintained until fix was completed
• Corrective action addresses the original deficiency
• Open follow-up items remain

After Photo Evidence

• After photo captured for the corrected condition
• After photo shows the same area and angle as the before photo where practical
• After photo clearly shows the completed fix
• After condition description entered
• Residual deficiency or limitation noted
• Photo comparison demonstrates measurable progress

Compliance Review and Closeout

• Before and after photos are paired and labeled
• Dates establish the sequence of events
• Documentation is sufficient for inspection or audit review
