  Record the test date, site or laboratory location, testing agency, contractor, and observer names.
• Calibration certificates for test equipment are current
  Confirm pressure gauges, flow meters, thermometers, and other instruments used for the test have current calibration documentation.
• Test setup photos and sketches are included
  Verify the package includes sufficient visual documentation of the mockup configuration, boundaries, seals, and instrumentation locations.

Air Infiltration Performance

• Air test method and pressure differential are documented
  Verify the report states the air test method, applied pressure differential, duration, and acceptance criteria.
• Measured air leakage rate
  Enter the measured air leakage rate for the mockup at the specified test pressure.
• Perimeter seals, joints, and transitions remained intact
  Confirm no visible failure, separation, or leakage occurred at perimeter seals, mullions, anchors, or interface transitions.
• Air leakage observations and locations of deficiency
  Document any observed leakage paths, sealant discontinuities, or non-conformances identified during the test.

Water Penetration Performance

• Water test method, spray rate, and duration are documented
  Verify the report identifies the water test method, spray rate, test duration, and acceptance criteria.
• Observed water penetration at interior face
  Indicate whether uncontrolled water penetration was observed at the interior face, joints, anchors, or interfaces.
• Locations of water intrusion or wetting
  Record the exact locations, extent, and probable source of any water intrusion, wetting, or leakage.
• Drainage paths, weeps, and end dams functioned as intended
  Confirm drainage components were unobstructed and performed without backflow or overflow.

Structural Performance

• Structural test load and duration are documented
  Verify the report identifies the applied load levels, load sequence, hold times, and acceptance criteria.
• Maximum measured deflection
  Enter the maximum measured deflection observed during structural testing.
• No glass breakage, fastener failure, or frame distress observed
  Confirm the mockup remained structurally intact with no cracking, loosening, pullout, or permanent deformation beyond criteria.
• Structural non-conformances and corrective actions documented
  Document any failures, temporary repairs, retests, or design revisions required to achieve compliance.

Thermal Performance and Envelope Continuity

• Thermal test method and acceptance criteria are documented
  Verify the report identifies the thermal evaluation method, environmental conditions, and acceptance criteria used for the mockup.
• Observed thermal bridging or discontinuity
  Indicate whether thermal bridging, insulation gaps, or envelope discontinuities were observed at anchors, spandrels, or transitions.
• Interior surface temperature or thermal metric
  Record the measured thermal value or other project-specified thermal metric used to assess performance.
• Air, water, and thermal interfaces are continuous at transitions
  Confirm continuity of the air barrier, water barrier, and thermal barrier at slab edges, anchors, perimeter seals, and adjacent construction.

Exceptions, Corrective Actions, and Approval

• List deficiencies and non-conformances
  Summarize all deficiencies, failed criteria, and deviations from the approved test plan.
• Corrective actions required before approval
  Select all corrective actions needed to resolve deficiencies.
• Final disposition
  Record the overall review outcome for the mockup performance test.
• Inspector signature
  Signature of the inspector or reviewer approving the completed inspection record.
