  Identify the specific space being entered, such as tank, vault, pit, manhole, trench access chamber, or similar confined space.
• Entry permit number or log reference recorded
• Date and shift of entry logged
• Entry type identified

Crew and Attendant Assignment

• Entrant roster documented
  List all authorized entrants assigned to this entry.
• Attendant assigned and present at the entry point
• Attendant maintains uninterrupted communication with entrants
• Rescue plan and emergency contact method confirmed with crew
• Competent person or entry supervisor identified

Atmospheric Testing and Air Monitoring

• Pre-entry atmospheric test completed before entry
• Continuous or periodic monitoring in place during entry
• Oxygen concentration within acceptable range
• Flammable gas/vapor reading within acceptable range
• Toxic gas readings documented for applicable hazards
• Atmospheric monitor bump-tested/calibrated per manufacturer requirements

Entry and Exit Tracking

• Entry time recorded for each entrant
• Exit time recorded for each entrant
• Headcount matches entrants currently inside the space
• No unauthorized entry observed
• Entry log updated in real time or at required intervals

Deficiencies, Corrective Actions, and Sign-Off
