  Enter the applicable site procedure, manufacturer instructions, or program reference used for the test.

Detector Identification

• Detector asset ID or serial number recorded
• Detector make and model recorded
• Sensor configuration verified for combustible gas monitoring
  Record the sensor type or channel used for combustible gas detection, such as %LEL.
• Last calibration due date checked
  Verify the detector is within the site's required calibration interval before use.
• Pre-use condition free of visible damage
  Housing, display, buttons, inlet, and alarm indicators show no cracks, missing parts, contamination, or other visible defects.

Bump Test Verification

• Bump test performed with approved test gas
  Confirm the detector responded to a known concentration of combustible test gas per the manufacturer's procedure.
• Test gas cylinder lot or batch number recorded
• Test gas concentration recorded
  Enter the certified test gas concentration used for the bump test.
• Detector response observed within acceptable time
  The detector must alarm or indicate response within the manufacturer's specified response time.

Calibration Check

• Calibration performed using approved procedure
  Use the manufacturer's calibration method or site-approved procedure.
• Zero reading before calibration
  Record the detector reading at zero gas before span adjustment.

Corrective Actions and Release to Service

• Deficiencies or non-conformances documented
  Describe any failed bump test, failed calibration, sensor drift, damaged components, or other issues.
• Corrective action taken
  Record repair, replacement, recalibration, removal from service, or escalation to competent person/vendor.
• Detector removed from service if failed
  Any failed detector must be tagged out and not used until it passes verification.
• Released for use after successful verification
