Bus Bloodborne Pathogen Cleanup Report

Inspection template for documenting cleanup of blood or other potentially infectious materials aboard a bus using a biohazard kit, including scene control, PPE use, decontamination, waste disposal, and exposure control plan follow-up.

Incident and Vehicle Identification

• Incident date and time recorded
• Bus identifier, route number, and location documented
• Type of material identified as blood or other potentially infectious material (OPIM)
• Cleanup performed by trained and authorized personnel

Scene Control and Exposure Prevention

• Passengers removed or kept away from contaminated area
• Contaminated area isolated and access restricted until cleanup completed
• Any sharps or broken items identified and secured without hand contact
• Exposure incident or splash to mucous membranes, non-intact skin, or sharps injury occurred

PPE and Biohazard Kit Use

• Appropriate PPE worn during cleanup
• Biohazard cleanup kit available and used
• Absorbent materials, disinfectant, and disposal bags used from kit
• PPE removed and discarded without self-contamination

Decontamination and Waste Disposal

• Visible contamination removed from all affected surfaces
• EPA-registered disinfectant used according to label directions
• Required contact time achieved before surface reuse
• Contaminated waste sealed, labeled, and disposed of per regulated waste procedure

Post-Cleanup Verification and Follow-Up

• Affected area inspected and found free of visible residue or odor
• Bus returned to service only after cleanup verification
• Exposure control plan follow-up required
• Supervisor review and inspector signature completed
