• Inspector name and role recorded
• Review scope includes discrepancy resolution and override events

Discrepancy Reconciliation

• All identified discrepancies are listed and matched to a resolution status
• Physical count matches system count after reconciliation
• Variance amount documented for each discrepancy
• Root cause identified or documented as unknown
• Resolution documented with date, time, and responsible party

Override Event Review

• Override events reviewed for the selected period
• Override reason documented and clinically justified
• High-frequency override patterns identified
• Overrides requiring pharmacist follow-up were escalated
• Override review completed by pharmacy and nursing leadership or designee

Access Control and Security

• Access to cabinet was limited to authorized users
• Manual access required pharmacy contact and was documented
• Any access exceptions were logged with reason and approver
• Cabinet remained secured after reconciliation

Compliance, Escalation, and Sign-Off

• Corrective action documented for each unresolved or repeated discrepancy
• Escalation to pharmacy director, nursing director, or compliance occurred when required
• Follow-up review date assigned
• Inspector attestation completed
