Log Details

• Record Date
  Date the usage log applies to.
• Shift
  Select the shift during which materials were used.
• Facility Area or Line
  Identify the production area, line, booth, or department where the materials were used.
• Recorded By
  Name or employee ID of the person completing the log.

Material Usage

• Materials Used Today
  Add one row for each product used during the day.
• Usage Details
  Capture product-level usage details for the audit trail.

VOC and Compliance Data

• Primary Product Type
  Select the main product type used most today.
• VOC Content Basis
  Select the unit used on the product label or SDS.
• VOC Content Value
  Enter the VOC content as listed on the product label or Safety Data Sheet.
• Is this product VOC-exempt?
  Indicate whether the product is claimed as VOC-exempt under applicable rules.
• Exemption Reference
  Provide the exemption name, rule reference, or product documentation reference.

Exceptions and Review

• Was there any deviation from normal usage or recordkeeping?
  Select Yes if there was a spill, substitution, missing label, unusual usage, or incomplete record.
• Deviation Type
  Select all that apply.
• Deviation Description
  Describe what happened, including the product involved and any immediate response taken.
• Corrective Action Taken
  Document any corrective action, notification, or follow-up needed.
