  EPA ID number for the transporter, if available.
• Destination facility name
  Name of the permitted treatment, storage, or disposal facility.
• Destination facility EPA ID number
  EPA ID number for the receiving facility, if available.
• Destination state
  State where the receiving facility is located.

Receipt Confirmation and Exceptions

• Receipt confirmed by destination facility?
  Indicate whether the signed manifest copy or receipt has been returned.
• Receipt date
  Date the signed copy or receipt was received.
• Was a discrepancy identified?
  Select yes if the returned manifest or shipment details do not match.
