Shift Details

  • Shift Date
    Select the date of the shift being logged.
  • Shift
    Choose the shift that best matches the work period.
  • Site / Area

  • Supervisor / Lead
    Optional. Enter the supervisor or lead responsible for the shift.

Associate and Activity

  • Associate Identifier

  • Activity Type
    Select the primary activity for this productivity record.
  • Other Activity
    Specify the activity when 'Other' is selected.

Production Output

  • Units Processed
    Total units completed during the measured period.
  • Lines Processed
    Optional. Enter the number of order lines, if applicable to the activity.
  • Labor Hours
    Total labor hours worked for the measured activity. Use decimal values where needed.
  • Engineered Standard UPH
    Enter the expected units per hour standard for this activity.
  • Engineered Standard LPH
    Optional. Enter the expected lines per hour standard when lines are tracked.

Exceptions and Notes

  • Was there downtime or a work stoppage?
    Select Yes if any interruption affected the measured productivity.
  • Downtime Reason
    Select all reasons that impacted the measured period.
  • Downtime Minutes
    Enter the total minutes of downtime during the measured period.
  • Variance / Audit Notes
    Add brief notes explaining any variance from engineered standards. Keep entries factual and work-related.
