Conference Details

• Resident Name

Notice and Invitation

• Notice Sent Date
  Select the date notice was sent to the resident and/or representative.
• Notice Delivery Method
  Select all methods used to deliver notice.
• Who Received Notice?
  Select all parties who received the invitation or notice.
• Was Timely Notice Confirmed?
  Confirm whether notice was provided within the required timeframe.
• Notice Verification Notes
  Add any details supporting the notice record, such as delivery confirmation or follow-up attempts.

Attendance Record

• Did the Resident Attend?
  Indicate whether the resident participated in the conference.
• Did the Resident Representative Attend?
  Indicate whether the representative participated.
• Attendees
  List attendees and their roles. Add one row per attendee.
• Attendance Summary
  Summarize who attended, who was absent, and any key participation notes.

Follow-Up and Audit Trail

• Is Follow-Up Needed?
  Select yes if any invited participant missed the conference or needs a rescheduled discussion.
• Follow-Up Actions
  Select all follow-up actions that were completed or are planned.
• Follow-Up Due Date
  Select the date by which follow-up should be completed.
• Follow-Up Owner
  Enter the staff member responsible for follow-up.
• Audit Trail Notes
  Record any additional details needed for compliance review, including attempted contacts and final disposition.
