Inspection Scope and Setup

• Platform identification and location recorded
  Record the platform ID, building/location, and inspection date/time.
• Inspection type selected
  Identify whether this is a pre-use, periodic, post-repair, or incident-related inspection.
• Manufacturer manual and site procedure available at point of use
• Inspector and crew briefed on hazards and rescue plan

Anchors, Tie-Backs, and Structural Attachment

• Anchorage points identified and rated for intended load
• Anchor hardware free of deformation, corrosion, cracks, or missing components
• Tie-back lines installed to independent anchors where required
• Structural attachment points show no signs of movement, damage, or unauthorized modification
• Anchor inspection date within required interval
  Record the date/time of the last documented anchor inspection.

Rigging, Hoists, and Suspension Lines

• Suspension ropes or wire ropes free of broken strands, kinks, birdcaging, abrasion, or heat damage
• Rope terminations, thimbles, clamps, and fittings secure and properly installed
• Hoists operate smoothly without abnormal noise, slipping, or uncontrolled descent
• Rigging components properly aligned and free from side loading
• Counterweights, if used, are secured and protected from displacement

Backup Safety Lines and Fall Protection

• Independent backup safety line installed for each worker as required
• Backup line shows no cuts, abrasion, UV damage, contamination, or broken fibers/strands
• Harness, lanyard, connectors, and rope grabs are compatible and properly connected
• Fall arrest attachment points and lifeline routing prevent swing-fall exposure

Controls, Limit Switches, and Emergency Systems

• Upper and lower limit switches function as intended
• Emergency stop control is accessible and stops movement immediately
• Control pendant, switches, and wiring are intact with no exposed conductors
• Emergency descent or rescue equipment available and serviceable

Platform Condition and Work Area

• Platform deck, guardrails, and toe boards intact and secure
• Access gates or entry points close and latch properly
• Work area below platform barricaded or controlled to prevent struck-by exposure
