Inspection Details

• Patient and room identified before audit
• Telemetry order and prescribed lead configuration verified
• Audit date and time

Lead Placement Verification

• Lead placement matches ordered configuration
• Leads are attached to the correct anatomical landmarks
• Lead wires are secure, untangled, and not under tension
• Electrodes are fully adhered and not lifting at the edges
• Electrodes are placed on clean, dry skin

Electrode Condition and Skin Prep

• Electrode gel/pad is intact and within usable condition
• Skin under electrodes is intact without redness, blistering, or breakdown
• Excess hair, oil, lotion, or moisture is addressed before electrode placement
• Electrode change interval documented per policy

Monitor Signal Quality

• Monitor displays an interpretable rhythm strip with minimal artifact
• No active lead-off alarms or intermittent disconnection observed
• Alarm limits are set per patient order and unit policy

Corrective Actions and Sign-Off

• Deficiencies documented with immediate corrective action
• Escalation to RN, charge nurse, or provider completed when indicated
• Inspector signature
