Exits and Perimeter Security

• All resident exit doors are secured and not propped open
• Delayed-egress or controlled-egress doors function as intended
• Door alarms activate when exit doors are opened
• Magnetic locks, latches, and hardware are intact and operating
• Perimeter gates, fences, or secured boundaries are intact

Corridors, Common Areas, and Walking Surfaces

• Corridors and exits are free of obstructions
• Walking surfaces are dry, even, and free of slip or trip hazards
• Rugs, mats, cords, and equipment do not create a fall hazard
• Furniture placement allows safe resident and staff movement

Fire and Emergency Life Safety

• Fire extinguishers are mounted, accessible, and not blocked
• Fire alarm pull stations are visible and unobstructed
• Emergency lighting and exit signage are illuminated and legible
• Emergency call devices or nurse call points are accessible

Resident Safety and Environmental Conditions

• Lighting is adequate in corridors and common areas
• Noise, clutter, and visual distractions are controlled
• Hazardous chemicals, sharps, and cleaning supplies are secured
• Resident care items and activity materials are stored safely

Corrective Actions and Sign-Off

• Deficiencies identified during the walk
• Immediate corrective actions taken
