Scaffold Daily Inspection

OSHA 1926.451 daily scaffold inspection with green/yellow/red tagging. Covers base/mudsill, frame integrity, planking, guardrails, ties, and access. Required before each shift by a competent person.

Inspection Details

• Inspection date and time recorded
• Scaffold location identified
• Competent person completed inspection before shift
• Current scaffold tag status

Base, Footing, and Support

• Base plates and mudsills are present, level, and properly bearing load
• Scaffold is plumb, level, and stable with no visible settlement or shifting
• Foundation or supporting surface is firm, unobstructed, and free of erosion or washout
• Screw jacks, leveling devices, and outriggers are secured and properly adjusted

Frame, Bracing, and Structural Integrity

• Frames, uprights, and braces are free from cracks, bends, corrosion, or other damage
• All pins, locks, couplers, and connections are installed and fully engaged
• Cross-bracing and diagonal bracing are installed per scaffold configuration
• No unauthorized modifications, missing components, or signs of overload are present

Planking and Work Platform

• Planks or decking are fully decked, secured, and properly overlapped or locked
• Work platform is free from broken, split, warped, or excessively worn planks
• Platform surface is clear of excessive debris, ice, mud, oil, or slip hazards
• Platform width and access path are unobstructed for intended use

Guardrails, Fall Protection, and Access

• Top rail, midrail, and toeboard are installed where required
• Guardrail components are secure and free from damage or excessive movement
• Safe access is provided by ladder, stair tower, or equivalent approved means
• Access points are clear, properly positioned, and free from climbing hazards

Ties, Anchorage, and Final Disposition

• Scaffold ties, braces, and anchors are installed as required by the configuration
• No adverse weather, overhead hazards, or nearby work conditions create a new deficiency
• Inspection result and tag status communicated to affected workers
• Inspector signature
