OASIS Resumption of Care Assessment (ROC)

Inspection template for verifying the OASIS Resumption of Care assessment is completed within 48 hours of a patient's return home from an inpatient stay of 24 hours or more, or on the ordered resumption date, per 42 CFR 484.55(d)(2).

Patient Return and Eligibility

• Inpatient stay of 24 hours or more documented
• Return home date and time documented
• Ordered resumption date documented when applicable
• Patient eligible for ROC assessment workflow

ROC Timing Compliance

• ROC assessment completed within 48 hours of return home
• ROC assessment completed on ordered resumption date when applicable
• Delay reason documented when ROC was not completed on time

OASIS Assessment Content

• Medication reconciliation completed and documented
• Current functional status reassessed
• Skilled needs and plan of care reviewed for changes after inpatient stay
• New or changed orders reconciled and documented

Documentation Quality and Follow-Up

• Assessment documentation is complete and signed by the clinician
• Any deficiencies or non-conformances identified
• Corrective action documented for any identified deficiency
