- No missing sections, slack conditions, or unauthorized removals observed
Openings, Holes, and Leading Edges
- All floor openings and deck penetrations are covered, guarded, or otherwise protected
- Opening covers are secured against displacement and labeled as required
- Leading edges are clearly identified and controlled to prevent exposure
- Materials, tools, and debris are not creating a trip, slip, or fall hazard near openings
- Guardrails, warning lines, or equivalent protection are intact around openings where required
Controlled Decking Zone Access
- Controlled decking zone is clearly defined and limited to authorized personnel
- Access points to the controlled decking zone are controlled or monitored
- Only trained workers with required PPE are present in the controlled decking zone
- Warning signs, barricades, or demarcation are visible and legible
PPE, Housekeeping, and Final Verification
- Required fall protection PPE is in use and properly fitted
- Housekeeping is adequate and walking surfaces are free of excess debris
- Any deficiency or non-conformance has been documented with corrective action assigned
- Inspector final risk rating
