Focus Four: Falls

- Scaffolds, platforms, and elevated work areas show no visible fall exposure deficiencies
- Fall protection training / rescue expectations reviewed with crew
Focus Four: Struck-By, Caught-In/Between, and Electrocution
- Traffic control, spotters, and exclusion zones are established for moving equipment
- Materials are secured to prevent falling, rolling, or shifting hazards
- Excavations, trenches, and confined work areas are protected from cave-in or entrapment hazards
- Temporary power, cords, panels, and tools are protected from electrical exposure and damage
- Crew reviewed lockout-tagout, energized work boundaries, and overhead utility hazards as applicable
现场 Readiness: Housekeeping, Access, and PPE
- Walkways, access routes, and egress paths are clear of debris, cords, and trip hazards
- Materials, waste, and tools are stored to maintain stable stacking and clear access
- Required PPE is available and worn for the task (hard hat, eye protection, gloves, high-visibility gear, hearing protection as applicable)
- Barricades, signage, and controlled-access points are in place where required
Emergency Preparedness and Site Controls
- Emergency contact list and site address are posted or readily available
- First aid kit, fire extinguisher, and emergency equipment are accessible and serviceable
- Emergency egress routes and muster point are known to the crew
- Weather, lighting, and site conditions do not create an immediate unsafe condition for work
- Any stop-work conditions were communicated and documented
Corrective Actions, Verification, and Sign-Off
- Deficiencies identified during the stand-down are documented
- Corrective actions include owner, due date, and priority
- Immediate hazards were controlled before work resumed
- Follow-up verification date assigned for open items
- Inspector signature completed
