Activation and Response Time

• Code Blue activated promptly after recognition
• Time from activation to first responder arrival
• Time from activation to crash cart arrival
• Defibrillator/AED available and ready for use on arrival

Team Communication and Role Assignment

• Team leader clearly identified
• Roles assigned within the first minute
• Closed-loop communication used consistently
• Team coordination maintained during CPR and equipment setup

CPR Quality

• Chest compressions started without unnecessary delay
• Compression rate maintained within target range
• Compression depth appropriate for patient category
• Minimized interruptions in compressions
• Ventilation technique appropriate when indicated

Equipment Use and Readiness

• Crash cart opened and organized correctly
• Defibrillator/AED pads applied correctly when indicated
• Airway equipment selected and used appropriately
• Equipment returned to ready state after drill
