Overview
PPE & Uniform Issue is a working register for recording who holds which safety kit or uniform, the size and type issued, when it was handed over, and when it should be replaced. Required fields are Item, Type, Issued on, and Status. Type options cover Footwear, Hi-vis, Gloves, Head, Eye, Hearing, Respiratory, and Other; Status options are In use, Due replacement, Returned, and Lost. Issued to identifies the person holding the item, while Replace by and Notes preserve the operational detail managers need.
The app uses an append-only posture: managers and admins add records, records are never deleted, and Status tracks the item lifecycle. That matters when an inspection or internal review requires evidence of what was issued, even after equipment has been returned or lost. Saved views separate In use, Due replacement, Replace within 30 days, and Lost items. The Overview screen provides a metric row and chart, Replace soon shows a calendar and cards, and Full register provides the complete grid.
Use this template when a site needs a controlled PPE handover and replacement register instead of a clipboard or spreadsheet. It is not a complete stock control or purchasing system, and it does not replace an organisation’s PPE risk assessment, fit testing, training records, or supplier process. The Monday 08:00 Weekly replacement list automation sends the app owner an AI summary digest titled PPE due for replacement based on the Replace within 30 days view.
Standards & compliance context
- The append-only register supports general occupational safety record-keeping expectations by preserving issue history rather than deleting returned or lost equipment records.
- Recording Type, Size, Issued to, Issued on, and Status can support an employer’s documentation of providing suitable PPE under general OSHA employer-provided PPE duties.
- The template should complement, not replace, risk assessments, equipment suitability checks, fit testing where applicable, worker training, and local retention requirements.
- Have the safety or compliance lead confirm whether additional fields or review steps are needed for the jurisdictions and PPE categories your organisation operates under.
General regulatory context for orientation only — verify current requirements with counsel or the relevant agency before relying on this template for compliance.
How to use this template
- Add the PPE & Uniform Issue app to your MangoApps workspace and confirm that the supplied fields, Type options, Status values, views, screens, and Monday digest match your issue process.
- Decide which managers and admins will add records and make status updates, since the register is intentionally restricted to controlled data entry.
- Record each handover as it happens by entering the Item, Type, Size, Issued to, Issued on, Replace by date when known, Status, and any relevant Notes.
- Use the In use, Due replacement, Replace within 30 days, and Lost saved views together with the Overview, Replace soon, and Full register screens to review current equipment.
- Read the Monday 08:00 PPE due for replacement digest, confirm the affected items and dates, and coordinate replacement or reorder action with the responsible manager.
- Close the lifecycle by changing Status to Returned, Lost, or Due replacement rather than deleting the original record.
Best practices
- Record the actual Size and Type for every issued item instead of grouping several items under a generic kit description.
- Enter Replace by dates at handover or immediately after the applicable inspection so the Replace within 30 days view can drive action.
- Use the Notes field for relevant handover context, such as a damaged item, temporary issue, or replacement decision.
- Change Status promptly when equipment is returned, lost, or no longer fit for use while preserving the original record.
- Review the Lost view separately from In use so a missing item is not mistaken for equipment still held by a worker.
- Assign one accountable manager per site or department to review the Monday digest and confirm that replacement action has been taken.
- Compare the Full register with physical stock checks periodically to catch unrecorded handovers, incorrect sizes, and stale wearer assignments.

Common use cases
Frequently asked questions
What does the PPE & Uniform Issue template track?
It records each issued item with the Item, Type, Size, Issued to, Issued on, Replace by, Status, and Notes fields. Type options include Footwear, Hi-vis, Gloves, Head, Eye, Hearing, Respiratory, and Other. The register shows who holds each item and whether it is In use, Due replacement, Returned, or Lost.
Who should add PPE issue records?
Only managers and admins add records in this template. This keeps data entry controlled while workers can still be represented in the Issued to field. Before rollout, agree which manager or administrator records handovers and status changes.
How often does the replacement reminder run?
The Weekly replacement list automation runs every Monday at 08:00. It checks the Replace within 30 days saved view and sends an AI summary digest titled PPE due for replacement to the app owner. It is a planning reminder, so managers should also update the Replace by date when an item is inspected or replaced.
Can this replace a paper PPE handover sheet?
It provides a working append-only register in your MangoApps workspace rather than a document or spreadsheet. Each issue remains recorded, and its lifecycle is tracked through Status instead of deleting the original record. This gives managers a clearer history when an item is returned, lost, or due for replacement.
Is this template a compliance record?
It can support employer-provided PPE record-keeping expectations, including documentation of issued equipment and replacement activity. It does not determine whether your PPE program meets every applicable occupational safety requirement. Review the fields and retention approach with your safety lead or compliance adviser.
What is a common mistake when using this register?
A frequent mistake is leaving Replace by blank or failing to change Status after equipment is returned or lost. Another is recording a general kit without noting the actual Size or Type. Require managers to complete the item, type, issued date, and status at handover, then update the record when its lifecycle changes.
Can we customize the PPE categories and statuses?
The template starts with the supplied Type options and Status values, which cover common PPE and uniform categories. Your team can review the setup during a demo and determine whether labels or fields need to reflect its issue process. Keep statuses mutually clear so the saved views continue to identify replacement, returned, and lost items correctly.
Does the template integrate with inventory or purchasing systems?
The supplied structure includes the register, saved views, screens, and the Monday replacement digest; it does not promise an inventory, purchasing, barcode, or other external integration. The Weekly replacement list gives the app owner a defined reorder input from items due within 30 days. Discuss any additional workflow requirements in a demo before rollout.
How should we roll this out to a site or department?
Add the app to your MangoApps workspace, assign managers and admins as record owners, and agree on one handover procedure. Enter current items with their wearer, type, size, issue date, status, and replacement date where known. Start reviewing the Full register and Replace soon screen each week, then use the Monday digest to plan replacement activity.
