Overview
Grant Reporting Preparation is a single-page procedure for building one traceable evidence pack for each grant reporting period. It opens by identifying four items to settle: the exact period, program evidence, the finance handoff, and review and submission ownership. A worked cover-sheet example uses Harbor Neighbors, a fictional community pantry preparing a January–March report for the Community Pantry Support Grant. Its sample records show 11 service sessions and 1,284 household visits, with two weather-related cancellations documented separately.
Use the page when program and finance contributors need a shared sequence for preparing report support, especially when activity records and expense summaries come from different roles. The comparison table distinguishes the questions each evidence stream answers, and the numbered procedure runs from confirming dates through review and authorized submission. A paired example contrasts a traceable cancellation log with a version whose gaps and totals cannot be checked. The closing section identifies the page owner, discrepancy path, key terms, and published policy links.
This is an example procedure, not a funder form, approval system, or submission tool. It does not decide which costs are allowable, define required metrics, or establish a grant's reporting cadence. Do not use the Harbor Neighbors figures as organizational results; replace sample details only after checking the award instructions and confirming local evidence owners, reviewers, and submission authority.
Standards & compliance context
- Follow the specific award instructions for reporting fields, definitions, evidence, deadlines, and submission route; this sample procedure does not establish funder compliance.
- Confirm applicable organizational policies and grant conditions with the program or finance lead for the relevant award, location, and reporting period.
- Preserve dated source records and explanations for gaps according to the retention and documentation rules that apply to the grant.
- Keep the intranet page readable and navigable with clear headings, descriptive link text, and accessible tables in line with WCAG 2.1 AA practices.
General regulatory context for orientation only — verify current requirements with counsel or the relevant agency before relying on this template for compliance.
What's inside this template
Assemble one evidence pack for each reporting period
Settle the reporting dates, evidence, finance handoff, and review route before contributors begin assembling the pack.
-
This page shows how program and finance contributors prepare the material behind a grant report so a reviewer can trace every statement to a dated record. The worked example is fictional: Harbor Neighbors , a community pantry program preparing its January–March report for the Community Pantry Support Grant . Always use the grant's own reporting instructions for required fields, definitions and the submission route. This page covers how to coordinate the evidence, not what a particular funder requires. Four things to settle, in order Reporting period: the exact start and end dates the grant uses. Program evidence: dated records that show what the program delivered. Financial handoff: what finance needs from the program team, and what finance prepares. Review and submission: who checks the draft and who submits it.

Program evidence and the finance handoff answer different questions
Keep program delivery records and financial summaries distinct while tying both to the same reporting period.
-
Keep them separate in the pack, but tie both to the same reporting period. Question it answers What did the program deliver? What did delivery cost, and where was it charged? From reporting period to submission
Follow the sequence from checking grant dates through collecting records, handing off to finance, and routing the draft.
-
The sequence Harbor Neighbors follows. This page describes coordination only; it is not an approval or submission system. Confirm the reporting period Check the start and end dates against the grant's reporting instructions, not last year's report. Write the dates at the top of the pack, and tell finance the same dates in writing. Assemble dated program evidence Collect the service-session log and volunteer shift sheets for that period only. Check that every entry has a date and a count. Add a one-line reason for each missing or cancelled session, with its supporting record. Hand off to finance Send finance the period dates, the supply and transport records for the period, and any questions about how costs were charged. Ask finance to name the figures it has prepared so the reviewer can compare them with the program log. Route the draft for review and submit Send the assembled draft to the designated program reviewer and finance reviewer. Resolve their comments in the pack, not in side conversations. Submit through the route the grant authorizes, through whoever the organization has designated to do so.
Explaining a gap: two versions of the same log
Compare a cancellation record that a reviewer can trace with one that leaves counts and missing entries unexplained.
-
Both versions below describe the same two cancelled sessions. A reviewer can only use the first. Traceable The log shows each cancellation with its date and reason. 4 Feb: session cancelled, severe weather warning; notice to households attached. 18 Feb: session cancelled, severe weather warning; notice to households attached. Household visit total (1,284) covers the 11 sessions held. Hard to review The log leaves blanks and the total cannot be checked. Two February weeks have no entry and no explanation. A total of 1,350 visits appears with no per-session counts. The report period in the pack differs from the one finance used.
Owners, discrepancies and where to raise them
Find the page owner, a practical route for resolving mismatches, and definitions for the evidence pack and handoff.
-
This page is maintained by Public Service and Program Operations . Raise corrections with that team, and ask for a review whenever a grant's reporting instructions or evidence ownership change. If the numbers don't match Compare the period dates in the program log and the finance summary first; a period mismatch can make the figures unsuitable to compare. Ask the program lead to confirm which records are the source for each count. Ask the finance lead to confirm which period and cost categories the summary covers. If ownership of a piece of evidence is unclear, ask the responsible program or finance lead before the draft goes to review. Definitions Evidence pack: the cover sheet, program records and finance summary for one report. Handoff: the point at which one role passes records or figures to another, with the period stated. The list below shows published organization policies you can read. Check with your program or finance lead which of them apply to your grant, location and reporting period.
Published company policies
Review available organization policies and confirm with a program or finance lead which ones apply to the grant.
How to use this template
- Check the current grant instructions and record the exact reporting-period start and end dates on the pack cover sheet.
- Assign a program contributor to gather period-specific service logs and supporting records, and ask site leads to verify dates and counts.
- Record each missing or cancelled activity with its date, reason, and supporting documentation instead of leaving unexplained blanks.
- Send finance the confirmed period dates and relevant program records, then ask finance to prepare an expense summary for that same period.
- Assemble the cover sheet, program evidence, and finance summary, and route the draft to the designated program and finance reviewers.
- Resolve comments in the working pack, confirm the authorized submitter and route from the grant instructions, and retain the final records under organizational policy.
Best practices
- Verify reporting dates against the current grant instructions rather than copying them from a prior report.
- Put the same confirmed period on the cover sheet, program logs, and finance handoff.
- Use dated source records for activity counts and identify which record supports each total.
- Document every cancelled or missing session with a reason and supporting record instead of leaving a blank entry.
- Keep program delivery evidence and financial records distinct while linking both to the same reporting period.
- Ask finance to name the period and cost categories covered by its summary before reviewers compare figures.
- Resolve discrepancies with the program or finance record owner before circulating the draft for review.
- Assign an owner to revisit this page when grant instructions or evidence responsibilities change.

Common use cases
Frequently asked questions
What does this grant reporting preparation template cover?
It covers the coordination work behind a grant report: confirming the reporting period, assembling dated program records, handing period-specific information to finance, and routing the draft for review and submission. The Harbor Neighbors example shows how to document cancelled sessions and connect counts to records. It does not define a funder's required fields or replace the grant's reporting instructions.
Who should use and run this procedure?
Program contributors assemble service records, with site leads helping verify session details, while finance prepares the expense summary. The example identifies a Program Director and Grants Accountant as reviewers. Your organization should confirm the actual owners and authorized submitter for each grant before using the page.
How often should we prepare an evidence pack?
Prepare a separate pack for each reporting period required by the grant. Confirm the exact start and end dates from the current grant instructions each time rather than copying dates from a prior report. If the funder changes its cadence or reporting requirements, update the pack process before the next cycle.
Does this template meet a specific funder's or regulator's requirements?
No. It is a coordination procedure, not an approved funder form or a claim of regulatory compliance. Use the grant's current instructions for required fields, definitions, allowable costs, retention rules, and submission route. Ask your grants or compliance lead to confirm any requirements that apply to your organization, location, and award.
What should we do when a session is cancelled or a record is missing?
Record the date and reason for the gap, then attach or identify supporting documentation, such as a cancellation notice. Do not leave blank entries that make it impossible to distinguish a missed record from an activity that did not occur. If the count or source record remains uncertain, resolve it with the responsible program lead before review.
How should we customize the example for our grant?
Replace the fictional Harbor Neighbors details with your grant name, reporting period, evidence types, role owners, and applicable review route. Keep program evidence and finance information distinct while ensuring both use the same confirmed period. Check the grant instructions and local organization policies before treating any sample field or process as applicable.
Does this page connect to finance systems or submit a report?
No connected finance, document, approval, or submission workflow is described in this page. It explains what contributors hand off and who reviews the draft; the organization must use its own approved tools and submission route. Add links or live blocks only after confirming their destinations and connections in the intranet.
How does a shared evidence pack compare with coordinating by email?
A single pack makes the reporting period, source records, gaps, finance handoff, and reviewer roles visible together. Side conversations can leave different versions of dates or figures in circulation, a pitfall illustrated by the hard-to-review example. Use the pack as the working reference and record resolved comments there, while following your organization's document-control practices.
How should we roll this out to contributors?
Confirm the grant-specific dates, evidence owners, reviewers, and authorized submitter before sharing the procedure. Walk program and finance contributors through the fictional example, then prepare a pack for the next reporting period using the grant's own instructions. Assign an owner to correct the page when reporting instructions or evidence ownership change.
