Start with what you observed
A concern is a reason to use the approved internal route, not a finding about a customer or another person.
Record only relevant, observable information using the organization’s approved process. Separate what you saw or heard from interpretation. Keep account and personal details within authorized systems; do not place them on this page or in informal messages.
Example distinction: “The customer paused twice and asked to speak without the accompanying person present” describes observations. “The accompanying person is exploiting the customer” is a conclusion and should not be presented as established fact.
Use the internal handoff
Follow the receiving organization's published procedure for local steps and any time-sensitive direction.
-
1
Notice and separate facts from inference
Identify the specific interaction or statement that prompted concern. Avoid labels, diagnoses, and conclusions. -
2
Keep communication neutral
Use open, non-leading language. Do not accuse, investigate, or promise a particular decision or outcome. -
3
Use the authorized escalation route
Consult the published internal procedure and send the concern only through its approved channel. Include only the information that procedure calls for. -
4
Follow the receiving role's direction
The authorized receiving role determines the next internal handoff under local procedure. Ask that role for guidance if the route or next step is unclear.
Keep wording factual and non-leading
These fictional examples show communication style; use the organization's approved procedure for the actual interaction.
Neutral observation
- Offers a choice without implying a conclusion.
- Does not promise a specific action or result.
Avoid assumptions
- States an unverified conclusion.
- Promises an outcome the staff member may not control.
Who owns the handoff
Worked handoff example
Coordination example only; no customer or account details.
A staff member notices that a customer repeatedly looks to an accompanying person before answering and asks to review information privately. The staff member does not infer why. Using the approved internal channel, they share the observable interaction and ask the authorized receiving role what the local procedure requires next. The page does not record the concern, determine an outcome, or replace the receiving role’s direction.
