Four things to settle before the review meeting
Preparation is complete when the approved evidence is assembled, every gap is a written question with an owner, each reviewer knows their role, and everyone understands that release is decided elsewhere.
Record completeness
Every page, entry, signature, attachment and referenced log is present and matches the executed batch record.
Open questions
Each gap is written as a specific question with the evidence it concerns, the role who can answer it and the date it is needed.
Review roles
The preparer, the quality reviewer and the laboratory contact each know what they check and what they hand on.
Release authority
The release decision belongs to the authorized role in the governed process. Preparation supplies evidence; it does not approve.
Preparation sequence
Illustrative order of work for sample lot HB-2417
Your approved procedure sets the required steps and timing. Use this order to keep the work coordinated.
-
1
Gather the executed record and its attachments
Collect the executed batch record and each referenced log, printout and report in one set. Check the page count against the record's own page numbering.
-
2
Check completeness against the matrix
Work through each evidence category below. Mark each one Complete, Open question, Waiting on owner or Not started.
-
3
Write each gap as a specific question
Name the record, the step or page, what is missing, and which role can answer. Avoid general notes such as 'logs incomplete'.
-
4
Route questions through the established channel
Send each question to the named role by your department's usual route. Do not alter a record to close a gap; corrections follow the approved correction practice.
-
5
Hand the set to quality review
Pass the assembled evidence and the list of questions with their current status to the quality reviewer, who confirms what is ready for the review meeting.
Evidence reference matrix: sample lot HB-2417
Authored sample as of 12 May 2025. It is not a system of record or a live work queue.
| Evidence category | Preparation check | Prepared by | Sample status |
|---|---|---|---|
| Executed batch record | All pages present; entries signed and dated; each correction shows who, when and why | Production | Complete |
| Material records | Component lot numbers on the record match the receiving records | Production | Complete |
| Equipment and cleaning logs | Each piece of equipment used has a log entry covering the run dates | Production | Open question |
| Laboratory results | Every result cited in the record has a matching reviewed report | Quality control laboratory | Waiting on owner |
| Deviations and changes | Each referenced deviation has a number and a recorded status | Quality assurance | Open question |
| Label and packaging reconciliation | Quantities reconcile and any difference is explained on the record | Packaging | Not started |
Open questions: a worked example
The same gap, written two ways
Too vague: 'Cleaning logs incomplete for HB-2417?'
Specific and answerable:
- Question: The batch record (step 14) lists Mixer 2 as used on 8 May, 14:10-17:40. The Mixer 2 cleaning log has no entry for the second shift on that date.
- Evidence concerned: Executed batch record, step 14; Mixer 2 equipment log.
- Needed: The log page for that shift, or the equipment owner's documented explanation.
- Asked of: The second-shift production lead, through the department's usual route.
- Needed by: The day before the quality review.
A reviewer can read the specific version and act on it without searching the record. The question closes when the answer is documented through the approved process, not when it is removed from the list.
Handoff order from preparation to release authority
-
Day 1
Day 1: Evidence assembled
Production gathers the executed record and attached logs and checks page counts.
-
Day 2
Day 2: Completeness check and questions raised
Contributors mark each evidence category and write each gap as a specific question with a named role.
-
Day 3
Day 3: Questions answered
Owners respond through the established channel. Answers are documented under the approved practice.
-
Day 4
Day 4: Quality review
The quality reviewer examines the assembled set and the status of each question.
-
After review
After review: Handoff to release authority
The authorized role receives the review outcome through the separately governed release process. That decision is not made on this page.
