Read the discussion and the next step
An outcome record separates what was discussed from what was decided or assigned.
A discussion does not itself mean a proposal was approved. A useful record distinguishes the topic, the clarification or discussion, and any next step with a responsible role. If an item is still open, say so plainly rather than implying it is complete.
Illustrative outcome record
Fictional example only; this is not a live forum record or a commitment.
| Record field | Example |
|---|---|
| Topic | Handover information across sites |
| Discussion | Participants compared where current guidance could be found. |
| Outcome | The coordinator will bring location differences back for clarification. |
| Owner | Forum coordinator role |
Interpret outcomes carefully
Clarification
A clearer explanation of an existing arrangement is not, by itself, a policy or process change.
- Look for the specific question that was clarified.
- Check whether the record identifies a source or responsible role.
Agreed next step
A next step should name a responsible role and describe the work to be done; it should not imply a guaranteed result.
- Find the stated owner role.
- Read the described action, not an assumed outcome.
Unresolved item
If no answer or next step is established, the record should keep the item visibly open.
- Look for a clear unresolved or open description.
- Do not treat discussion alone as completion.
